Care Mobility Rewards Program

The SKCMC and Access to Healthcare Committee are planning the innovative Care Mobility Rewards Program

In August 2019, Hopelink Mobility Management was awarded a grant by the National Center for Mobility Management to launch a pilot program that provides transportation for Medicare-eligible older adults in South King County. The South King County Mobility Coalition and the King County Mobility Coalition’s Access to Healthcare Committee are partnering to produce a pilot designed to reduce barriers for patients to access post-discharge health care appointments and to help them avoid readmission to hospitals. At the time of their initial discharge, Medicare patients identified as being at a higher risk for re-hospitalization will be enrolled in the Care Mobility Rewards Program. Clients will be given credits that can be redeemed for round-trip transportation to qualifying follow-up care needs, such as doctor appointments, pharmacy trips, or physical therapy.

In 2015-2016, Hopelink partnered with coalition members and advocates to create a business plan for a program that ensures transportation access is not the reason for hospital readmission. This 2019-2020 grant will test elements of the plan and launch a 3-month pilot from February 2020 to until April 2020. The grant runs through May 15th, 2020.
