Care Mobility Rewards Pilot Launches!

The pilot is underway from February 18th - May 8th!

The Care Mobility Rewards Pilot launched on Tuesday, February 18th, 2020! The Care Mobility Rewards program is an incentivized transportation pilot that provides free transportation for post-discharge appointments to Medicare patients who have been recently discharged from Valley Medical Center. This program is a partnership between Valley Medical Center and Hopelink’s Medicaid Brokerage and Mobility Management departments.

The pilot runs from February 18th until May 8th and is available for Valley Medical patients who have been recently discharged and have Medicare and a higher risk of re-admission (LACE+ score 60 and above). Under this limited 3-month pilot, the patients will receive free transportation for their discharge, medical appointments, and up to 5 roundtrip trip for their wellness visits. Some sample wellness visits include, but are not limited to: senior center lunches, pharmacy visits, grocery shopping, smoking cessation class, exercise classes etc. This pilot is meant to deliver a number of valuable qualitative and quantitative data to be evaluated.
